Paperwork Needed:
You must provide a medical history statement and minor consent form (if applicable) prior to trip / excursion. **NOTE: If anything is marked yes, requires a licensed medical professional’s endorsement authorizing your ability to perform scuba diving activities prior to any water training. If you are not able to participate in scuba diving activities due to medical reasons and you have already signed up for class, Professional Diving Addicts LLC or affiliates are not responsible. Please check with your licensed medical professional prior to signing up. One medical form per person required. Deposits are non-refundable**
